Most annual plans are dead by February because a year gives you permission to defer. A 12-week plan removes that permission: twelve weeks is long enough to achieve something meaningful and short enough that every single week is visibly load-bearing.

This template is structured the way The 12 Week Year by Brian P. Moran and Michael Lennington prescribes: a small number of goals (never more than three), each broken into specific weekly tactics, and a weekly execution score — the percentage of tactics you actually completed. Score 85% or better most weeks and the goals take care of themselves.

What's inside this template

Two goals, not seven

The template ships with two goal slots on purpose. The most common failure mode in 12-week planning is carrying an annual plan's ambition into a quarter — five goals means none of them get the weekly attention that makes this system work. Pick the one outcome that would make the quarter a win, add at most one supporting goal, and park everything else. In Pilot the backlog is one click away, so 'parked' never means 'lost'.

Tactics you can score on Friday

Every tactic in this template passes the Friday test: at the end of the week you can answer 'did I do it?' with yes or no. 'Improve outreach' fails the test; 'make 10 outreach touches a day' passes. Frequency-based tactics (the 5/week counters in the preview) are the workhorse — they turn intentions into lead measures you control completely.

The scorecard is the product

The weekly execution score — completed tactics ÷ planned tactics — is the one number this template exists to produce. The 12 Week Year's benchmark is 85%: below that, results become unpredictable; above it, they compound. The template renders the score after every week so a bad week is a signal you catch in days, not a quarter-end autopsy.

Week 13 is part of the plan

The template ends with a built-in 13th week: review the quarter, take the win, and plan the next 12 weeks while the lessons are fresh. Most people skip this and lose two weeks of momentum between quarters — the 13th week is how consecutive quarters chain into a genuinely better year.

How to use it

    Set your 12-week goals

    Choose 1–3 outcomes for the next 12 weeks. Each needs a measurable lag measure — a number that will be true or false on the last day.

    Write weekly tactics for each goal

    For each goal, list the 3–5 recurring actions that produce it. Each tactic must be binary-checkable at the end of a week.

    Run your week from the scorecard

    Each week, work from the tactic list — not your inbox. Check tactics off as you complete them.

    Score every Friday

    Your weekly execution score is completed ÷ planned tactics. 85%+ means on track. Log what blocked anything you missed.

  5. 5

    Review weekly, reset at week 13

    Hold a 15-minute weekly review (or a WAM with your accountability partner). After week 12, use the 13th week to review the quarter and plan the next one.
